My suggestion would be to verify that the libGL is not the default libGL.
For the short term, fglrx will continue to require it's own libGL.
LIBGL_ALWAYS_INDIRECT may also provide some exxtra information.

Another thing to check that the usage count on the kernel module is
non-zero.

Rather, try upgrading to the latest 6.8.2. I see in your log that your X
server says it's version 6.8.1.99.

Doesn't his fglrx listing mean he's got a problem with the install?
