When using xine in clone mode, the window appears on both screens, but
the video output only comes through the primary screen, the secondary
stays dark inside the window. Can I fix this?
No, it's a hardware limitation. You can put the video overlay on the
secondary monitor on server startup using
Option "OverlayOnCRTC2"
How do I switch the mouse/keyboard between screens in dual head mode?
If the screens are adjacent in the server layout, you can move the
cursor between them over the adjacent edge.
Can I run two separate instances of X with different XF86Config-4's?
In general yes, but that's not well tested/supported with fglrx.
