Not 100%, but it indeed usually doesn't at that point.

Yes.

debian system?

The fglrx packages should contain (or depend on) everything necessary.

If they are installed, are there other possible failures that prevent 3d
acceleration from working correctly?

Yes. The output of

LIBGL_DEBUG=verbose fglrxinfo

should indicate what's wrong.
